Registration Application
All sessions have limited enrollments.The online registration form below is provided for your convenience. If you are remitting by cash or check or if you prefer not to send us your credit card information through our secure server, the information entered here will be returned on a printer friendly page after you click the "OK" button at the bottom of the form. Please print the resultant page on your local printer to mail or fax back to us.
Fees - Workshop fees must be sent with the registration form. Pay by check, payable to Conferences and Institutes or by VISA/MC only. Refunds will be given with cancellation 2 weeks prior to the beginning of the workshop, but will incur a $30 processing fee. The University reserves the right to cancel these workshops due to insufficient enrollment and in the event this occurs, students will be given a full refund.
Enrollment is limited and prepaid registration is required. Please use a separate registration form for each registrant. You may pay by check or credit card. (No cash through mail.)
